Instructor forms & information - Electronic-Paper

Use "Electronic-Paper" forms to request copyrighted and non-copyrighted material that is in an electronic or paper format be put on reserve. (Examples include: journal articles, book chapters, websites, entire books, government documents, lectures notes, old exams, etc.)

Note: When submitting "printable" forms, a "Course Information" form must accompany the submission requests each time.
